Cookie baking contest entry categories:

• Employee Department Entries are FREE
• Individual - $5 per entry
• Business - $10
• Professional - $10

Prizes will be awarded in each category.

Proceeds benefit medical student scholarships.

Turn in form to the foundation office at 201 NW R.D. Mize Road, Blue Springs, Missouri 64114; or email to mmaddox@carondelet.com; or fax to 816-655-5371 by November 9. Cookies will need to be turned in November 13 by noon. Bring one dozen and your entry fee to the information desk at the main entrance to St. Mary's Medical Center
